D5SR-FORM 2025 - Certification of Fingerprint Technician
- Requires signature of FC and DCDR.
- In the Adobe Reader software, uncheck the box “lock form after signing”.
- E-mail as an attachment to DCDR.
- Submitted to DIRAUX by DCDR.
D5SR-FORM 2025 - Class Attendance Roster (Non-National Workshops)
- Only used when a record of a non-national workshop is needed.
- Form is not submitted to DIRAUX and typically not IS Office
D5SR-FORM 2025 - Death of Member Notification
- Form can be prepared by FSO-HR or another member
- but is signed and submitted to DIRAUX with ANSC-7035.
- Submitted to DIRAUX by FC.
- FC also sends notifications to DCDR, DCAPT, & DCO
D5SR-FORM 2025 - DIRAUX Waiver Request
- Waiver request is initiated by FC, then submitted up the CoL/M to DCDR, DCAPT, and possibly DCO.
- In the Adobe Reader software, uncheck the box “lock form after signing”.
- Appropriate justification for waiver must be included with request.
- For example, request for 3rd year as FC or DCDR must include what will change during the next year to resolve the leadership situation.
- Note AUXMAN references on form
- FC initiates for Flotilla waiver, DCDR initiates for Division level.
- Typically, DCAPT will submit to DIRAUX

D5SR-FORM 025 - Member ID Card Application (30APR25)
- Member ID Card Application is for renewals and reissues only.
- Form must be complete including current weight, height, hair color, eye color, and blood type.
- Note box with photograph instructions. DO NOT CROP PHOTOGRAPHS.
- Submitted to DIRAUX by FC
D5SR-FORM 2025 - Removal of REYR due to AUXCT
- FC must verify in AUXDATA II or AUX-Directory that member is current in AUXCT Certification.
- Check all boxes that member is currently in REYR due to AUXCT REYR.
- There could be additional reasons that member is in REYR:
- lack of hours,
- lack of activity,
- lack of workshop, etc.
- Submitted to DIRAUX by FC.

D5SR-FORM 2025 - Removal of REYR due to Lack of Uniform Inspection
- FC must enter or verify that a Uniform Inspection has been entered into AUXDATA II,
prior to submitting this REYR removal request form.
- Submitted to DIRAUX by FC
D5SR-FORM 2025 - Qualification for Vessel Examiner (VE) or Recreational Boating Safety Visitor (RBSV)
- FC verifies that member has completed the VE or RBSV examination and conducted mission activity under supervision in AUXDATA II
- Submitted by DIRAUX by FC
D5SR-Form 2025 - Surface PPE Request Form - Rev 05-25
- One form per member ID number, no group orders.
- Member works with FSO-RS or SO-RS to determine appropriate PPE necessary for position.
- Boat Crew Trainees no longer receive PLBs until certified.
- FC reviews order, signs and submits to DIRAUX.
D5SR-FORM 2025 - Voluntary Request for Termination of Certification
- Used when a member voluntarily terminated a certification, this a better than staying in REYR for 5-years until certification archives.
- Submitted to DIRAUX by FC

- The National AUXOP Program Worksheet that shows members the various paths to AUXOP Status membership.
- Submitted to DIRAUX by FC
D5SR-FORM 2025 - Credit for Qualifications
- This form is used is to certify that an active-duty or recently retired USCG individual has met training and qualifications attained and current during their USCG service with copy of verification documents (please attach copy of certificate or transcript) in order to be granted the equivalent qualification in the CGAUX
