Auxiliary Member ID Card Application Form
1. Download USCGAUX Member ID Form.
2. Fill out the form and click on the "Submit Form" button.
3. When the email with the form attached comes up you may add a square shaped picture with a red background. The member must be in uniform.
4. Name the JPG file with your member number. ie 1234567.jpg.
5. You need to cc the email to yourself for safe keeping.
