2. CGAUXA-10  - Consent to Membership Form
   
   
3. Copy of Birth Certificate / Copy of U.S. Passport (Current / Expired)
   
   
4. Dues Check (Amount will be provided by the Staff Officer - Human Resources)
   
   
5. Blue Pen - as all forms and signatures are to be done in blue ink, if possible please have one for any other requirements that are needed during the enrollment process.
