Change of Membership Form 7035 Checklist

The 7035 form is used for various changes, most often disenrollments, retirements, and deaths. The 7035 in 8WR is to be routed to the DSO-HR, not directly to the DIRAUX. The DSO-HR will check for accuracy and forward when complete. There are certain requirements not noted in the instructions specific to 8WR. This checklist should help in insuring procedures are followed.

General Instruction:

Always use the most current 7035 form found in Forms Warehouse.
These forms CAN be emailed, it is not necessary for original signatures as an Enrollment Application
Always complete Name and Member number
Please always record Flotilla in Section II (example 085-05-06)   

Section I

Completed by Flotilla Commander
Follow Flotilla Standing Rules for dues notification to members.
This section is completed and sent to flotilla member in the event they have not responded to request of dues payment.
This should be sent to member by 01NOV at the latest. Allowing 30 days to respond.   

Section II

Completed by Member appropriately to their desires
It should be returned by 01DEC or the member is subject to possible disenrollment
If they choose to disenroll, use codes on second page to best describe reasons why    
Signature and return of ID card and all CGA equipment is
expected for those disenrolling or retiring. 

Section III  

Completed by Flotilla Commander
Enter effective date
CHECK ONE either non-payment of financial obligations or Request (IT CANNOT BE BOTH)
Flotilla Commander Signature and date. 

NON-PAYMENT WITHOUT SIGNATURE:

Requires an attached addendum stating the means and number of attempts to contact the member
Various methods should be
employed to contact (email, mail, phone calls etc.)
Dates/copies should be provided, if possible, of all attempts 

AT MEMBERS REQUEST WITHOUT SIGNATURE:

Requires an attached addendum stating it is understood that is the desire of the member
Addendum must state how that became the understanding, (i.e., personal conversation, email, phone call,
text,) Include date and person(s) contacted by the member requesting to leave active membership.     

Death Of Member

Death of members is to be reported to DIRAUX through the Chain of Leadership ASAP.
Complete 7035 in a timely manner after reporting the death to DIRAUX
Include next of kin contact on the form and add a copy of obituary, if possible, as an attachment 

RETIREMENT WITHOUT SIGNATURE

Requires an attached addendum stating it is understood that is the desire of the member
Addendum must state how that became the understanding, (i.e., personal conversation, email, phone call, text,) include date and person(s) contacted by the requesting member
Include date of membership found in the Directory or Auxdata.

Email or mail completed form to DSO-HR
