FORM ANSC 7020A (04/19) - Photography/Video/Audio Consent Form / Release

We must have a signed release form for all minors in photos. We cannot use any image of a minor without a signed consent form if ANY part of the face (including the side) or identifying part of the minor is visible. This includes photos in public venues, group shots, and schools. The forms may be emailed in advance to get parents signatures for scheduled events. It helps to keep a folder or clip board of blank 7020A forms available when taking photos at events.
