National Forms houses all PDF, electronic (e-forms) and web forms for use by Auxiliary members. Also houses the FAQ section for those unfamiliar with certain forms. Check here for the most up to date versions of forms and other documents.

For non-mission hours, log directly into AuxData II, which has replaced webform (7029). Use this link to log your hours each month and it will submit to your IS officer.

Mission Activity Report (7030) For mission hours. (PDF Email form) The Lead should use this to log hours for all participants and submit to the appropriate IS officer.

Team Coordination Training - Risk Assessment Worksheets 

The General Assessment of Risk (GAR) form has been updated to the GAR 2.0. All members should take the updated training in learning how to use this.  This is mandated for use in District 13!
