This Is the procedure (see below) used for ANY mishap involving Auxiliarists while performing official duties whether under orders or “assigned to duty” such as VSCs, PVs, etc. The critical ingredient not mentioned here is that the activity must be related to Auxiliary duties and prior notification of the activity to the FSO or VFC/FC is part of the documentation process that will be required when reporting a mishap.
Note: The procedure in the Aux Manual is outdated and incorrect. This document is correct and brings the instruction up to date. Document everything as soon as you can after the mishap because memories fade with time.

YOU notify the Flotilla Commander as Soon as Possible by phone with details. Then Emails to:
1. Flotilla Commander, Luis Cribeiro, 404-735-2741c [email protected]
a. He is then to notify the DIRAUX, Director of the Auxiliary Miami Fl., Coast Guard Commander . Officer Harry Mautte; # 305-415-7052; Email is [email protected]
b. Then notify the DCDR, Div. Commander, and John Sprague-Williams, 706-473-1013c Email is [email protected]2. Now DCDR John S/W, will contact DCO, District Commodore, Judith Hudson, #305-332-6085c 305-967-8163h; Email is [email protected]
a. He will also contact DCAPT-N, Pat McMenamin, #706-830-7370c; Her Email is; [email protected]3. DCAPT-N, Pat McMenamin will check with the DIRAUX just in case not notified.
a. DCAPT-N will then notify DCOS, District Chief of Staff, Gary Barth # 772-321-3041c; Email is [email protected]
