Pilot/Aircrew Qualification Form (7015)

Here is the Pilot/Aircrew Qualification Form (7015) that is required of all Pilots, Aircrew, and Observers at least once per year.

I suggest that you download the file, fill it out, and then save it on your local hard drive.  Next year just make a copy and then make any changes required.

An initial pilot package consists of

  1. correctly filled out 7015 that is signed by an authorized FE
  2. a filled out USCG Auxiliary Pilot Qualification Checklist that is signed by an authorized FE. the checklist is found <here

 


Aircrew/Observer Medical Screening form

Aircrew Medical Screening form is used by Aircrew and Observers.  Observers and Aircrew can optionally have a FAA Class III medical. An FAA Medical is normally gotten by Auxiliary members who also have a pilots license but who do not yet qualify for one of the Auxiliary pilot designations. (co-pilot, first pilot, aircraft commander)

Facility Inspection Forms

A complete package that needs to be submitted to DirAUX is:

1) 7005 - Offer for use
2) 7005 D8 addendum
3) Facility information Sheet
4) Picture of oil analysis log entry
5) Picture of Oil analysis report
6) Picture of Annual Logbook entry (airframe, engine, & Propeller)
7) PPE Inspection Report

Here are the forms that need to be used to complete your facility inspection.  

Note: opening the PDF files in a browser WILL NOT WORK.  You MUST download the files to your local hard drive and fill them out using Adobe Acrobat Reader.

PPE Inspection Report (PDF).  The Inspection report is NOT a form.  you need to print it out and fill it out by hand.

Note 1: You must download the form and use Adobe Acrobat Reader to fill the form out and to save it.

Note 2: This version of the Facility Information Sheet allows the picture(s) to be added to the form when you are using Acrobat Reader.  However, the built in instructions imply you can't.  Use the separate instruction sheet.  

Note 3: It is important that you send the filled out and saved FILE to your AAC. It is not possible to add the pictures to a scanned or printed version.

A separate authorization letter is no longer required.  Authorized Pilots are noted right on the 7005


Statement of Understanding for Equipment Issue.

When your facility is approved you may be issued PPE for use in the plane.  For the most part it is:

1) a personal floatation device for each usable seat

2) a emergency locator beacon. This will need to be registered with NOAA

3) a raft for each usable seat

4) a couple of other odds and ends

Note: "usable" seat is the number of positions in the airplane that you can fill and still maintain proper weight and balance.  For Example, My C182 only has 3 usable seats.  if I assume 220lb per person,  then I am over max gross weight if I have 4 people in the plane and full fuel.

 

here is the form that you will be required to sign acknowledging that you are receiving the gear and that you will return it when requested or done with it.
