Assignment To Duty - COVID-19, Phase II

Due to COVID-19, all Assignment To Duty(ATD) requests must be preceded by an approved 
7101(7-20) Auxiliary COVID-19 High Risk Certification.  

If you already have an authorized Auxiliary COVID-19 High Risk Certification on file, proceed to Step 3, otherwise start with Step 1.

    • Download and print out form
    • Complete form
    • Sign and date form

STEP 2:  Please submit completed form to D11-SMB-D11AUX-South@uscg.mil.

Please note that a new Aux-COVID-19 High Risk Certification Form does not need to be submitted with each request for assignment to duty as long as the member states that the form has been submitted and there are no changes to the member’s status.
