The Boys Scouts of America and the U.S. Coast Guard Auxiliary have developed a joint photo/video/audio release form for BSA members.

Coast Guard Auxiliary Photo Policy Regarding Children

The U.S. Coast Guard Auxiliary has issued photography guidelines for taking pictures of minors. Auxiliary publication editors do not want to discourage taking great kid photos; it's just that Auxiliary photographers need to make sure they have parental permission (including a signed photo release).
