Boating Safety Forms

In the menu bar to the left, the you'll find links to download and print the boating and boating safety forms described below. These forms can help make you a safer boater and, in extreme circumstances, save your life!.

Please note that if a link has an asterisk (*) in front of the description below, in using this link you will be leaving the Flotilla 22-5 website and that the USCG Auxiliary makes no claims about the security, policy or standards of the linked sites.

  • * Accident Report Form: If you are involved in a boating accident, you may need to report the accident to the Coast Guard. The general criteria for reporting is: death or disappearance of a person, damage in excess of $2000 or a person requiring medical treatment beyond first aid. The Coast Guard's form CG-3865 should be used to report these accidents. (6 page PDF, 8 1/2 X 11. May be filled out on-line, then printed or printed and filled out by hand.)

  • Checklist: Developed by the National Safe Boating Council, the Boater & Boat Equipment Checklist helps ensure you've got what you need when you go out on the water. This form covers not only the boat; it covers you, the boater. In addition to providing a list of basic safety items, it offers suggestions for upgrades as well. It is designed for vessels under 26 feet in length. (2 page PDF; 8 1/2 x 11)
