Common Forms Used by Members

The primary forms used by members of our flotilla are:

E-form used to report most common activities. These include [indicates code for that activity]:
  • Traveling to/from and attending Flotilla Meetings [Member-99E, Officer-99A]
  • Studying for and taking online courses [99D]
  • Studying and working with your mentor for Boat Crew & Coxswain Certifications [99D]
  • Travel to/from Flotilla Operations (Waterway Watch, Boat Patrol, ATON Verification, Vessel Examinations) [99B]
  • Travel to/from RBS Visits[99B]
  • Travel to/from Direct Support to the Coast Guard [99E]
  • Officer activities [99A]

To access the 7029 e-form: CLICK HERE, then:

  1. Log in with your Member Zone credentials.
  2. Under "7029 Form Access" select "7029 - Version 1.1".
  3. Select an "Active" report from the dashboard and hit "Open", or create a new report by hitting "New".
  4. Enter your information. Be sure to hit "Save" at the bottom of the page before exiting.
  5. When you are ready to submit the report hit "Submit".
It is suggested that you put a month's worth of activities on a report before submitting.

An 7030-Individual-Mission Code 31 is used by ATON Verfiers (AVs) to report # of PATON aid discrepancies & aids watching properly, and time spent filling out 7054s as well as preparing photographs. Mission time for all participating members, including AVs, is recorded separately by the Mission Leader.

An 7030-Individual-Mission Code 07D is used to report hours in direct support to the Coast Guard as well as maintaining websites

An 7030-Individual-Mission Code 07V is used to report hours in support of America's Waterway Watch program

Used to report monthly Recreational Boating Safety (RBS) Program visits and hours

Used by AV-qualified members to verify PATONs. May be used by any member to report the status of Federal or Private ATONs. (Note: this is the version approved by ANT Ponce de Leon)
