Core Training Self-Attestation Form

USCGAUX Division 7, Narrangansett Bay members who complete the mandated Core Training Courses through personal study and/or localized training can receive completion credit for this training by completing the official Core Training Self-Attestation form.

Download the form, then please read, understand, and sign this document.

Once completed, Attestation Forms should be sent to the Cape Office for entry; do NOT send this form to your Member Training (MT) or Information Services (IS) Staff Officer. For scanned forms, send the form via encrypted email to D01-SMB-D1-S-AUX-Support-CTR@uscg.mil. For hard copy, send via USPS First Class or Priority mail to:
